What RN PocketPal does
- Scenario-first.Speak or type one or two sentences (“CHF exacerbation, 78 y.o., SOB on 4L, lower-extremity edema”); the AI returns 2-3 prioritized NANDA diagnoses.
- PES format. Each diagnosis is built as Problem · Etiology · Symptoms — the format your clinical instructor / preceptor is grading against.
- SMART outcomes. Specific, measurable, achievable, relevant, time-bound. Defaults to short-term (end of shift) and long-term (24-72h) outcomes per diagnosis.
- Interventions with rationale.Each intervention pairs the action with the rationale (so you don’t lose points for a naked verb).

Frequently asked questions
- Is the AI output graded by my instructor?
- No — your instructor grades what you submit. Care Plan Builder produces a draft. You edit, refine, and own the final submission. Schools differ on AI-assisted work policies; check yours.
- Are the diagnoses NANDA-approved?
- Yes — the bundled library uses NANDA-I 2024-2026 diagnostic labels. The AI prompts also constrain the model to NANDA labels.
- Can I add my own diagnoses to the library?
- Not in v1. Submit a Suggestion (Profile → Suggestions) and accepted additions ship in updates with the next dataset refresh.
- Does it work offline?
- The 20-diagnosis bundled library works offline. AI scenario expansion needs connectivity.
- Why nursing-only diagnoses?
- Care plans are a nursing-scope artifact. The tool intentionally won’t produce medical diagnoses, prescriptions, or treatments outside RN scope of practice. That keeps it useful to your instructor and safe at the bedside.
