Does Note Writer send patient data to OpenAI or DeepSeek?
No identified patient data. Names, MRNs, DOBs, and room numbers are scrubbed on-device before the request leaves your phone. The proxy server itself never logs request bodies. The AI sees a de-identified clinical description and the format you chose.
Which note format should I use?
DAR for routine event documentation; SOAP/SOAPIE for full assessments; PIE for problem-focused; Narrative for free-text shift summaries; Focused for short single-issue charting. The app picks a reasonable default from the event template; you can always override.
Can I edit the templates?
The 12 event templates are designed by working RNs and locked for v1 so the defensible-format guarantee holds. Free-text editing of the AI output is unlimited.
Does it work offline?
The format scaffolds, voice dictation, and local draft list work offline. The AI rewrite step requires connectivity. Without it, you still get a structured template to fill in manually.
Is this a substitute for charting in my EHR?
No. Note Writer produces a draft you copy into your EHR. The official record lives in your facility EHR; this is the scratchpad that gets you there faster.
