An AI note writer that respects your license.
Draft DAR, SOAP, SOAPIE, PIE, Narrative, and Focused notes in seconds — with PHI scrubbed on-device before any AI request, and every output reviewed by the clinical team that designed the templates.
Charting is the slowest hour of every shift. Writing a defensible DAR or SOAP that actually says what happened — without copy-pasting from a template that doesn’t fit — eats the breaks you don’t have. Generic AI writers either leak PHI to a third party or produce notes that read like AI wrote them.
Note Writer was designed by working bedside RNs across telemetry, ICU, ED, and med-surg. Twelve event templates (admission, fall, code response, restraint initiation, IV insertion, blood transfusion, change in status, refusal of care, etc.), six note formats, voice dictation, and PHI scrubbed on-device before anything leaves your phone.
What RN PocketPal does
- Six formats.DAR · SOAP · SOAPIE · PIE · Narrative · Focused. Each with built-in section prompts so you don’t miss the parts that matter for audit.
- 12 event templates. Admission, discharge teaching, fall, code, restraint, IV start, blood transfusion, change-in-status, refusal of care, family update, medication error, isolation-precaution handoff.
- Voice dictation. Speak the raw description; the AI rewrites it into the chosen format. You keep editorial control — the draft never auto-saves to the EHR.
- PHI scrubbed on-device. Patient names, MRNs, DOBs, room numbers are stripped before the AI request leaves your phone. The proxy server never sees them.
- Defensible by design. Templates encode the components that survive a chart audit: time-stamped observations, objective findings before subjective interpretation, escalation chain, response to interventions.
- Local-only history. Drafts stay on your device — nothing backs up to the cloud without your explicit opt-in.
Who it’s for
Bedside RNs, LPNs, advanced-practice nurses, and nursing students who want defensible documentation without losing their breaks. Designed for shift documentation; the output is meant to be the draft you refine and paste into your EHR — never a substitute for clinician judgment.
Frequently asked questions
- Does Note Writer send patient data to OpenAI or DeepSeek?
- No identified patient data. Names, MRNs, DOBs, and room numbers are scrubbed on-device before the request leaves your phone. The proxy server itself never logs request bodies. The AI sees a de-identified clinical description and the format you chose.
- Which note format should I use?
- DAR for routine event documentation; SOAP/SOAPIE for full assessments; PIE for problem-focused; Narrative for free-text shift summaries; Focused for short single-issue charting. The app picks a reasonable default from the event template; you can always override.
- Can I edit the templates?
- The 12 event templates are designed by working RNs and locked for v1 so the defensible-format guarantee holds. Free-text editing of the AI output is unlimited.
- Does it work offline?
- The format scaffolds, voice dictation, and local draft list work offline. The AI rewrite step requires connectivity. Without it, you still get a structured template to fill in manually.
- Is this a substitute for charting in my EHR?
- No. Note Writer produces a draft you copy into your EHR. The official record lives in your facility EHR; this is the scratchpad that gets you there faster.
