How to review NCLEX rationales. The minutes after each question are where you learn
Read every option's rationale. The right answer: Say why it's right in your own words. Each wrong answer: Say why it's wrong or unsafe for this client. Then reread the question: Find the word that made the difference

STEP 1

1. Read every option's rationale

  • The right answerSay why it's right in your own words
  • Each wrong answerSay why it's wrong or unsafe for this client
  • Then reread the questionFind the word that made the difference
Tag every question. Knew it: Right, and you can explain why. Lucky: Right, but you guessed between two. Gap: Wrong because you didn't know the content. Misread: Wrong because you missed a word like first or except

STEP 2

2. Tag every question

  • Knew itRight, and you can explain why
  • LuckyRight, but you guessed between two
  • GapWrong because you didn't know the content
  • MisreadWrong because you missed a word like first or except
Fix the tag, not the score. Lucky or Gap: Go back to the content for that topic. Misread: Restate the question in your own words before you read the options. Knew it: Move on. That one is done

STEP 3

3. Fix the tag, not the score

  • Lucky or GapGo back to the content for that topic
  • MisreadRestate the question in your own words before you read the options
  • Knew itMove on. That one is done
Keep a miss log. Date and Client Needs area. What the question was really asking. The rule you missed, in one line. The day you'll retest it

STEP 4

4. Keep a miss log

  • Date and Client Needs area
  • What the question was really asking
  • The rule you missed, in one line
  • The day you'll retest it
Retest within a week. Redo your missed topics a few days later. Use new questions, not the same ones. Watch your Lucky tags shrink

STEP 5

5. Retest within a week

  • Redo your missed topics a few days later
  • Use new questions, not the same ones
  • Watch your Lucky tags shrink
