A nursing student brain sheet for clinical day.
Pathophys notes, med rationale fields, lab interpretation prompts, and SBAR scaffolding — shaped for the assignment your instructor actually grades.
- Two patients, four diagnoses each, six meds you've never seen.
- A care plan due before clinical with pathophys, rationale, and three diagnoses.
- SBAR report that feels like an oral exam.
What RN PocketPal does
- Per-patient pathophys note field.
- Med rationale fields that prompt you to think through the why.
- Lab interpretation prompts (what does this value mean for this patient).
- SBAR scaffolding that walks you through Situation, Background, Assessment, Recommendation.
- Care plan scaffold linked to NANDA-style diagnoses.
Who it’s for
For Nursing students (ADN, BSN, accelerated BSN, LPN/LVN, student CNA).

What a nursing student should put on a clinical brain sheet
- Primary diagnosis and the pathophysiology in one line
- Each med with its class and why THIS patient is on it
- Abnormal labs and what they mean for your care today
- Nursing diagnoses you can defend with your own assessment
- Questions for your instructor, written as they occur
- Skills you want to get signed off this shift
Want one you can print?
Two free options, no signup: a ready-made printable brain sheet (PDF) with a single-patient sheet and a 4-patient grid, or the brain sheet builder if you want to choose the layout and how many patients fit on a page.
Paper works on units that do not allow phones. The digital version in the app is for everyone else.
Frequently asked questions
- Will this replace my instructor's care plan template?
- No — it scaffolds the structure, but you fill in the clinical reasoning. Transcribe into your program's required format for grading.
- How do I protect patient information?
- Patient initials and brain-sheet contents stay on your iPhone in encrypted local storage. Best practice in school: use initials only.
- Is this an NCLEX prep tool?
- No. RN PocketPal is a clinical-prep tool, not an NCLEX prep tool. NCLEX prep is a separate product category we deliberately don't compete in.
- Can I use it after I graduate?
- Yes. Switch your role from 'Student RN' to 'RN' in your profile and the brain sheet, calculators, and rhythm drills stay with you.
